If you arrive oustide of normal business hours, please text 651-243-1227 and I will virtually unlock the door for you! Once inside, please have a seat in the guest lounge and I will be with you as soon as I'm done preparing the studio for your appointment.

I want you to feel as comfortable as possible during your time with me! I provide drinks & snacks as well as other essentials such as glasses protectors, phone chargers, disposable headphones, aromatherapy and more! If there is anything you need, please don't hesitate to ask.

By checking this box I agree to the Salon Cancellation Policy: To ensure every guest has an exceptional experience, and to respect scheduled appointment times of all guests, I understand that appointments cancelled or rescheduled with less than 48 hours notice may be subject to a 100% cancellation fee, but no less than 50% of the cost of scheduled services. I agree that this fee will be charged to my card on file with Cybele Styles at her discretion. This INCLUDES missed and no-show appointments. Punctuality Policy: I understand that in the occurrence of a late arrival, services will be adjusted according to the service time remaining and that I will be charged in full for my originally scheduled service. Arrival Policy: Upon arrival for my appointment, I will enter the salon, check in for my appointment via the virtual check-in kiosk, and follow the prompts. I understand that if I arrive early, I may have to wait in the guest lounge until my scheduled appointment time to begin my services. Appointment Adjustment Policy: I understand that refunds will NOT be given on services rendered. If I am unhappy with the results of my service, I agree to reach out to my stylist within 7 days of my original appointment to schedule an adjustment appointment. Retail Refund Policy: I understand that I may exchange any opened or unopened retail product within 14 days of purchase. My stylist will suggest an alternative product to meet my needs and the value of the product I am returning will be applied towards the new product. I understand that cash or credit back for returned retail products is NOT an option and that retail returns can NOT be used as a service credit. Confidentiality and Privacy: I understand that every client has the right to privacy and confidentiality and that the conversations I have with Cybele Styles will remain confidential. I understand that my personal information is confidential and will not be sold to or shared with any outside companies.

By reading and agreeing to this statement, I have read and understood all of Cybele Styles' Policies found in the Digital Welcome Packet and stated in this form. I agree to have my payment information saved and stored securely. I also understand that Cybele Styles and I will have an in depth conversation speaking about all aspects of today's appointment, in which I agree to and acknowledge, including price, and the expected physical and aesthetic results. I hereby release and agree to hold Cybele Styles harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Cybele Styles. I understand that this release discharges Cybele Styles from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Cybele Styles. This liability waiver and release extends to the salon together with all owners, partners, and employees. *
